Health Assistance
We will provide a medical assistant in the form of a CNA or EMT to help in care management of the patient following the orders of an Emergency Room discharge doctor for the short stay of the patient in our residential setting
Case Management
We will provide Care Management in the form of a Public Health social worker to help the patient navigate resources for temporary housing, food, and access to other resources.
Community Health Workers
We will provide a Community Health Worker that will assist in health education, health navigation, screening, and assessments, as well as individual support as the patient, goes on their health journey. They will be able to continue this communication through our platform even after the patient leaves our Smart Medical Home facility.
